MKMG has made some exciting improvements to its website at www.mkmg.com. The site now includes on-line forms for scheduling appointments and requesting prescription refills, as well as a host of other information and resources.
“This is a very exciting development for all our patients,” said Christopher Sclafani, Chief Operating Officer. “Many people use the internet as part of their daily routine at work as well as at home. These web-based services allow patients to contact us at their convenience, without having to call the office or wait to speak to a nurse. We are confident that these new services will quickly become popular with our web-savvy patients, and may also help to reduce the volume of phone calls coming into the Group each day, as well as hold times.”
For Existing Patients Only: Appointment and Rx Refill Requests
The Appointment Scheduling Form may only be used by existing patients who are already receiving treatment from an MKMG physician. New patients who are not registered with MKMG should not use the web-based form to schedule first-time appointments. Patients must provide three appointment options (three different days/times desired) in order to submit their request. The on-line form is NOT for urgent visits -- on-line requests must be submitted at least 5 days in advance of the first desired appointment date. MKMG will confirm the appointment by E-mail or telephone within two business days.
Existing patients may also use the Prescription Refill Request Form to request that a regular prescription be refilled by their primary care physician. Information required includes: name of medication, dosage, frequency (how often taken) and pharmacy name/telephone number. MKMG will confirm your prescription refill request within two business days via E-mail or telephone. However, patients should call their pharmacy before going to pick up their medication, to make sure their prescription is ready. This form cannot be used if you are requesting any change in the name or dosage of your medication.
New Patient Registration Forms
New patients can access MKMG’s On-Line Registration Form to simplify paperwork during their first appointment. This form cannot be submitted on-line; it must be printed out and completed. and a hard copy given to the nurse at the time of the first appointment. Reminder to new patients: please bring a printed copy of your registration form with you, or you will be asked to fill out a new form at the time of your visit. |
